¹	You will find the order number on the top, right hand corner, of your bill of lading. When keying the order number, the leading zeros on member number or sequence number are not required.
²	Your last name, origin phone number or origin postal code is used as your password to grant you access to your Survey Form. You need to enter one of these three values.
